POSITIVE ID FOR DONORS

Beginning January 30, 2004, donors will be required to provide an acceptable form of ID to
donate.

The Preferred identification is one of the following:

  • American Red Cross Donor Card (any version or date)

  • Current United States or foreign government issued photo ID
    *Drivers License
    *State ID
    *Passport
    *Military ID
    *INS (Immigration and Naturalization Service) card ("green card")

If a Red Cross blood donor card or picture ID from the above is not available, two additional
forms of ID such as those listed below are required.  The ID must contain the donor's name,
at minimum, and be current.

Work ID                                                                  Credit card or bank card
Social Security Card                                              Library card with donors name
Birth Certificate (original or certified copy)       Drivers license without photo
Personal check book with name and address   Professional license (for example, RN, LPN)
Non Red Cross donor card                                  Voter registration card
School ID or College ID                                        Selective service card
Grocery store frequent shopper or VIP              Club or museum membership card
         with donor's name at minimum
Vehicle registration                                               Insurance card
Fishing or hunting license                                   Payroll stub

 

Unacceptable Identification

  • piece of personalized mail

  • bank statement

  • utility bill, and

  • second party verification (Exceptions: high school student donors at a drive
    held at their school or religious affiliations)
