POSITIVE ID FOR DONORS Beginning
January 30, 2004, donors will be required to provide an acceptable
form of ID to
donate.
The Preferred identification is one
of the following:

American Red Cross Donor Card
(any version or date)

Current United States or foreign
government issued photo ID
*Drivers License
*State ID
*Passport
*Military ID
*INS (Immigration and Naturalization Service) card ("green
card")
If a Red Cross blood donor card or
picture ID from the above is not available, two additional
forms of ID such as those listed below are required. The ID
must contain the donor's name,
at minimum, and be current.

Library card with donors name
Birth Certificate (original or certified
copy) Drivers license without

Personal check book with name and address Professional
license (for example, RN, LPN)
Non Red Cross donor

with donor's name
