REGISTRATION INSTRUCTIONS
Deadline-February 2, 2004 for all courses
1. All Participants: Complete VTI Registration Form &
Checklist (on back)
2. Disaster Courses: Complete & sign 5898H for each
course. A completed 5898H must be submitted for EACH course. Please
do not send one 5898H attached to a registration for multiple courses.
It is acceptable to fill out the personal information section of the form
and photocopy, then fill out the title of course, and training and experience
prerequisite portions as appropriate for each course. This expedites processing
and approval of applications. The staff thanks you!

Please review the requirements in the course descriptions before
submitting your application!
Confirmation: Upper level courses must be canceled if minimum
numbers are not met by the deadline. For this reason, we ask that you
register by the deadline. You will be notified if your class is canceled.

FEES
Nonrefundable administrative fee of $15 per registrant.
Course enrollment fee is $20 per day. The fee for each evening session
is included in that day's fee, or a $5 fee is charged for the evening
session when no daytime course is attended on that day.
Please note that certain courses require additional fees, as noted in
the course description. Additional fees for materials cannot be refunded
if a participant withdraws from a course after the deadline.
Course registration fees may be charged to major credit cards.
Registrations will not be processed without accompanying payment.

FINANCIAL ASSISTANCE
DISASTER SERVICES: Funding may be available for current VA/DC DSHR Specialists
or higher in the leadership track who are willing to serve on the State
Level III & IV Teams.

REFUND POLICY
· There is a $15 nonrefundable administrative fee per registrant
· Additional fees for materials are nonrefundable.
Registration refunds will be approved only if one of the following conditions
exist:
· A class is canceled by the Institute.
· An application for a course is denied.
· A participant is assigned to a disaster relief operation.
· A participant withdraws before February 25, 2004, through a written
request to the Institute.
Refund requests will be considered after the close of
the 2004 Virginia Training Institute.
